Tech Exploration Vaccine Confirmation

We’re excited to meet you on Monday, July 12! For those participating in person, we would like to confirm vaccine status so we can ensure the safety and comfort of everyone attending.  Please complete this form prior to the start of the program.

    Per UW policy, in order to go maskless, you must be fully vaccinated. Those who do not confirm their vaccinated status as "fully vaccinated" must wear a mask while indoors at the Steve Ballmer Building.
